According to the probable cause affidavit filed on March 10, Medicaid Fraud Control Unit Investigator David G. Meachum was assigned to investigate the matter.
Meachum explained, “During the course of the investigation I obtained information that Haywood had allegedly submitted fraudulent time sheets to Public Partnerships LLC, the financial intermediary for services provided to Irene Matthews, while Matthews was inpatient at two area facilities.”
Haywood, an unlicensed respite care worker, was hired by Matthews’ family to provide in-home care to Matthews, the court document explains.
Haywood was paid through funds disbursed by the Indiana Medicaid system and managed by Public Partnerships LLC.
During the period of Oct. 9, 2007 through Nov. 16, 2007, Matthews was an inpatient at Glenburn Home and Greene County General Hospital. Haywood submitted time sheets indicating that she provided services to Matthews during this time frame, states the court document.
Haywood was not eligible to receive payment as Matthews was an inpatient.
On Jan. 21, 2008, Haywood submitted a time sheet dated Oct. 14, 2007 to Public Partnerships LLC for 40 hours of services provided.
On Jan. 21, 2008, Haywood submitted a time sheet dated Nov. 11, 2007 to Public Partnerships LLC for 97 hours of services provided.
“It is documented that Haywood submitted time sheets for payment of 137 hours during the period which Matthews was inpatient, at the rate of $9.79 per hour, for a total loss to Medicaid in the amount of $1,341.23,” notes the court document.
Matthews’ family members denied signing the time sheets submitted by Haywood.
However, one family member did explain that she had provided several blank time sheets, which she had signed, to Haywood for her to fill out in the event that she was not available, states the court document.
But the family member also contended that the time sheets submitted fraudulently were not signed by her, alleging that Haywood signed her name to the time sheets.
Both family members signed their respective signatures for comparison to the time sheets — which neither appeared to be the signature on the time sheets.
Haywood was employed by Matthews’ family from fall 2007 through January 2008. She provided care for Matthews in Linton.
A warrant for Haywood’s arrest was issued March 10 and she was picked up and booked in Greene County Jail on March 11. She was released after posting bond that same day.
